Question: Hair Fall Problem Dry Dandruff Congential, Need to prevent conversion of Testosterone to DHT. How o do it safely.

Brief Answer: Finasteride; Minoxidil; Hair transplantation Detailed Answer: Hi there. I read your brief but clear history & expectations. A safe way of doing what you desire is to use a tablet called Finasteride 1 mg/day. It does have its side effects but these are negligible & I have tons of patients' on this medication & none to date (over the last 15 yrs) have developed any of the serious side effects including decrease in libido...so rest assured. In addition, for the interim period, one may use topical 2% minoxidil solution of any brand; I favour Neoxidil or MX. Dry dandruff is a slightly more pesky issue..please try a stay on ketoconazole lotion applied at nights & left on. XXXXXXX is the name of one such lotion in India. In addition a mild anti-dandruff shampoo eg Danclear plus left on wet scalp for 10-15 mts contact twice/week may help..give it a try for 2 weeks. Hi Doc Thanks for responding to the query. I have earlier used ketoconazole lotion but to no benefit as such. Though it provides a temporary relief but just temporary. I am using a mild shampoo everyday (herbal, khadi make). Earlier I used using Lotus Anti-dandruff shampoo (for 2 years, Sep-2011 - Dec 2013). Tht was quite good to use. What could be the side effects for Finasteride, negligible it may be as its going to work on one's hormonal balance. I am asked by a hair stylist to use shampoo daily to have a clear scalp free of dust or oil (which may be generated during the course of the day). For how many days do I need to take - Finasteride (1 mg/day). I underwent Mesotherapy (2.5 months, 9 sessions in total, 1 session per week) at VLCC and then suddenly stop (should have done it gradually). It was good till that time...but. I used to have wet dandruff before sept 2011, Now since washing everyday, they have become dry, I do not use hair oil for lubrication (once in a week, put badam oil or jhonson baby oil during night and wash it in the morning, do not keep it for more than 12 hours) Please suggest things in more details and to the queries above.

Brief Answer: Pick a dermatologist nearest you; Patience key! Detailed Answer: Heya XXXXXXX I understand your anxiety but these 'hair-raising' issues that go into detail will require a one-on-one conversation & may require at least a half hour to an hour of a consult. I suggest picking a dermatologist/trichologist near you in Delhi for the same & feel free to use our HCM portal to help you identify one of your choice. Please take the word of a registered medical practitioner & not a hair stylist is my honest straightforward opinion with no dis-respect intended to the latter. Use a shampoo of your choice ONLY twice/week but let the contact time be 10-15 mts as I suggested earlier on a wet scalp. Lotus seems to have worked for you. Yes you should have staggered the mesotherapy & it is at best an 'adjunct' therapy. Before you start finasteride, please do a semen count/analysis as this is the most worrisome side effect. It has to be taken for a minimum of 4-6 mths to observe some good result & under the supervision of a dermatologist ..hence my suggestion to pick one & stay the distance with him/her. Relief in dandruff is usually temporary; don't expect too much; try this new formulation of XXXXXXX 'stay-on' lotion...acts better & 'stays on ' overnight.
