Question: I have had a headache accompanied by a very cold nose for about four days. If I cover my nose or get it temporarily warm, the headache eases, but return as soon as my nose becomes cold again. What causes this and what is the treatment?

Thank you for your query.

1. It is important to differentiate the cause for the cold nose and the headache.

2. If the headache has preceded the cold nose, then the cold nose is explained by the vasodilatation and loss of heat. In this case, the headache is more likely to be a Migraine or a Trigeminal neuragia.

3. If the environment is very cold, then vasoconstriction and congestion in the nose may be causing internal structures to touch, causing pain and headache.

4. A mild Raynaud's Phenomenon has to be ruled out if there is any change in color of the nose or cold extremities, in addition. Many conditions (especially autoimmune) and medications can cause this.

5. Get a Neurologist's opinion, a Sinus CT scan and blood tests done. Based on these results, further treatment can be planned.

6. meanwhile, request your doctor to prescribe a trial with a nasal decongestant, an anti-migraine medication and to review any current medication (including side effects of Tramadol).
