Brief Answer:
Your most of symptoms are due to fibromyalgia and migrain.

Detailed Answer:
Hi, dear
I have gone through your question. I can understand your concern.
Your symptoms like pain, memory loss, lack of concentration, confusion, fatigue tingling and numbness, cognitive disorder all are related to fibromyalgia.

Headache, blurring of vision, sensitivity to light all are related to migraine.

You have low sugar, vitamin B12 and vitamin D level also. That also affect this condition.

Vitamin b12 deficiency leads to tingling, numbness and cognitive disorders.

Your treatment is very complicated. It need multidisciplinary team approach. You need treatment from physician, neurologist and psychiatrist.

You should go for MRI brain and neuropsychiatric evaluation. Then you should take treatment accordingly.

Drugs like pregabalin, duloxetin amitrytiline are useful in fibromyalgia. Paracetamol and tramadol can be used for pain relief.

You should also take vitamin B12 and vitamin D supplements.
These all are prescription based medicine and need monitoring.
Consult your doctor and start treatment accordingly.
