Question: Adult 34 yo daughter fell. 9x9 inch very deep bruising / hematoma entire right buttock and peri region and part of left buttock. (and L4 bulging disc per MRI) What should treatment be on day after fall?

Traumatic injury can lead to bruise due to hematom

Detailed Answer:
Hi,

I have studied your case.

Traumatic injury can lead to bruise due to hematoma formation.
Clinical examination is important to rule out ligament injury and spine injury.

For these symptoms continue analgesic and medication like chymotrypsin can be started.
You may apply thromophobe ointment over bruise.
Till time take medication like analgesic and apply crepe bandage if possible over thigh, if required supportive splint can be used. Do ice fomentation and have good rest for early healing.

Unless the large hematoma is compressing a nerve or you can do cold fomentation.
Very rarely we need to drain hematoma, usually it resolves gradually on own improving healing process.

If required you may need to do blood investigation for prognosis and recovery.
Colour Doppler study will help to know any vascular injury.
Take precaution to avoid infection around hematoma.
MRI spine will be helpful to diagnose spine injury along with it see foe coccyx and sacrum injury.
