Suggest Treatment For Hematoma On The Cheek

default
Posted on Mon, 21 Sep 2015
Question: I fell 14 days ago and landed on my right cheek. A very large hematoma formed and has not gone away. I applied ice for the first 48 hrs. and then switched to heat as per directions from my e.r. visit. The blood has coagulated and is very hard. Ive tried rotating warm and cold compresses. I went to a dr. on Monday and said it just required time. I'm starting to panic because i'm a teacher and return to work on Tuesday of next week. Any advice would be welcome.

Brief Answer:
Kindly upload an Image

Detailed Answer:
Hello. Thank you for writing to us

I have gone through your query and I have noted down your concern.

A hematoma usually resolves by itself. Any surface color changes that are part of a hematoma would usually take 2-3 weeks to resolve.

I suggest you to take an Oral Vitamin C supplement once daily.

However, I request you to kindly upload an Image so that I am able to guide you better.

You may upload an Image either at "Reports Section"

Or

You may mail me at YYYY@YYYY with the subject line as Attn: Dr. Kakkar.

Also let me know has it reduced Or is it still the same size? Is it painful?

Regards

I"ve been doing vit. c and applying vit K cream externally for days now. Yes, it has reduced in size, it's just very solid, and large. I wouldn't say it's painful, it just feels as if a golf ball has been shoved under my skin. I will send a photo when my son comes home.

Is that info helpful?

Brief Answer:
I suggest conservative management for another 3-4 weeks

Detailed Answer:
Hi.

Since it is hard that suggests that it has begun to organise.
You may wait another 2-4 weeks to see if it reduces by itself simply because it was a large hematoma initially and will take time to shrink
You may take an Oral anti-inflammatory e.g diclofenac preferably in combination with a serratiopeptidase to help resolve edema and inflammation.
Warm compresses might be useful by increasing local circulation and helping it resorb.
An ultrasound Or MRI scan will show the exact size of hematoma and give more details about the hematoma.
If the hematoma still does not resolve significantly even after 3-4 weeks, it may then require a minor surgical procedure to take it out.
