Hi.
Thanks for your query, albeit short and specific.
To recapitulate: Male/35 - blood and mucus on toilet paper - diagnosed with hemorrhoid 5 years ago - groin and thigh soreness recently...

If there is blood and mucus on the toilet paper, there is more chance of having procto-colitis.
Hemorrhoid per se do not cause any mucus production.
Groin and thigh soreness can be due to dermatitis and better be seen by a Dermatologist.
For the blood and mucus on toilet paper, I would advise you the following:
Please consult a General Surgeon for clinical evaluation, examination of the abdomen and more importantly Ano-proctoscopy with per-rectal digital examination to see for any probable cause, the degree of hemorrhoids and to plan for further management.
Colonoscopy may be needed.
Tests of stool, blood and urine as routine.
The treatment will be as per the causes found.

It shows peri-anal excoriation too.
Please get the stool test done and if there is an evidence of an infection, you may be given a course of an antibiotic and metronidazole or so, as per the choice of your Doctor.
The part on the left side of the image is not very clear.
Please post a clear and focused image.

The additional photos show:
Probably lax anal canal
Prolapsing piles - to be confirmed by proctoscopy.
There looks to be a bleeding spot in one of the images.
Peri-anal excoriation is more.

The picture makes me think of excoriation, cracked skin, masses which need evaluation by a General Surgeon/Proctologist as well as needs a second opinion of a Dermatologist. Proper treatment should be initiated at the earliest as already explained in details.
