Suggest Treatment For High Fever And Severe Loose Stool In A Child

Question: Hello Doctor,
My son 3yrs 8months old, is having fever since monday (i.e., 29th June). now its been 5 days he's still getting fever.
He came back from school on Monday & was absolutely fine but suddenly by evening he got fever 101.2 deg F. We gave him Crocin DS as suggested by our pediatrician then after few hours fever came again & we gave Ibugesic. this continued whole next day, he was not willing to have food & was throwing it out. We consulted our pediatrician, he said its Viral & had dry cough too, so gave cough syrup for the same. Then from wednesday loose motion started, whatever he ate or drank just went out, we started Econorm. Then late wednesday night he did vomit & loose motion was severe, so we started with Antibiotic as prescribed by our pediatrician, he gave 5 days Antibiotic course. Then from thursday evening his loose motion stopped & he didn't vomit again but fever was still there & cough turned to wet cough.
Today (i.e., Friday) he was not waking up from bed in morning, somehow I woke him up & he was feeling too weak to stand or sit just wanted to lie down on bed. Somehow in afternoon he had some food though he was feeling very hungry but wasn't able to get up & eat. By evening he started coughing again & got fever again, I gave him antibiotic & now fever is not there.

Thanks for the query

The initial presentation of fever with vomiting,cough and loose stool is suggestive of viral infection.But fever due to viral infection should start decreasing after 72 hours and the general well being should also get improved.
Sometimes even in viral infection the fever may persist for longer duration but the severity and frequency should be decreased.
In view of persistent fever,cough and poor general condition I would like to get a hemogram and CRP done to see whether there is any secondary bacterial infection.A chest Xray is also needed to see any involvement of lower respiratory tract.
If the reports are normal I won't get overly worried as bacterial infection has been ruled out and would like to continue the same medication for 3-4 more days.If report is suggestive of bacterial infection then I would like to add antibiotic like azithromycin to cover atypical organism.
Encourage him for regular intake of healthy/nutritious and easily digestible foods with plenty of fluids to combat the weakness.
