Posted on Mon, 27 Feb 2017
Question: Hi, I am a 61yo male with a transplanted Kidney (6 years ago) and with diabeties caused by the transplant medicine. My symptoms appear to be occasional hot-flashing, and intense sleepiness around 2-3pm every day. Drugs are the Prograf/Myfortic/Prednisone protocol for the transplant and Lantis/Humalog for my diabetes. What can I do to get some relief for both/either the hot-flashing and/or intense sleepiness at work. Thanks

Brief Answer:
Possible causes

Detailed Answer:
Good day,
Noted your question. Thank you for contacting us. Hotflushing can happen due to couple of reasons in diabetes. 1) Autonomic dysfunction causing gustatory sweating.
2) Hypoglycemia. 3) Low sexual hormones causing hot flushes.

Do you think you have sweating episodes while eating or while resting which lasts for few minutes? Have you ever checked your blood glucose levels when the hotflushes/sweating episodes occur?
How is your libido/erection?

It is possible that you may be developing post meal hyperglycemia/hypoglycemia after lunch which is causing the excess sleepiness between 2-3 pm. Could you please do a blood glucose levels at that time period using your glucose monitoring device? Based on that, we can advice further. It is good to take a power nap ( after setting an alarm in your watch) of 20-30 minutes maximum post meals which is shown to improve the efficiency at work place.

It may be a good idea to do a thyroid function tests, CBC and a fasting Testosterone level ( before 9 am in the morning) to assess further.

regards
Binu
