Question : type 2 diabetic wife, age 55, has had a blood sugar spike up to around 210 and has been nauseous and vomiting consistently for last 8 hours. Complains of pain across shoulders. Tingle in fingers. Blood pressure is high at 209/129. She has an appointment with her regular doctor at noon today. it is 3:30 am now. Question is, safe to wait to see regular doctor or go to emergency room?

I carefully passed through your question and would explain that the high blood pressure and the spike in blood sugar are a physiological reaction to the pain.

Coming to this point, I would recommend bringing her to the ER as soon as possible for a physical exam and some tests to exclude possible serious causes underlying her pain (like a dissection of the thoracic aorta, esophageal spasms, coronary artery disease, pulmonary embolism, etc.):

-a chest X ray study
-a resting ECG and a cardiac ultrasound , coupled with cardiac enzymes
- some blood lab test (complete blood count, liver and kidney function tests, blood electrolytes, cardiac enzymes, D-Dimer levels).

Further tests (angio CT scan of the chest, coronarography) may be needed based on the above test results.
