Suggest Treatment For Hypertension While On Bisoprolol And Quinapril

default
Posted on Thu, 4 Feb 2016
Question: I have never had a blood pressure problem. Always 120's over 70's. I do take Bisoprolol in PM and Quinapril in AM since heart attack in 2004 - just never instructed to stop. I have COPD and have a habit of taking oxygen levels and BP every morning. Last few months BP has been VERY high upon waking (180-200 over 100-110) and low before sleep at night around midnight. In AM I take a xanax and wait an hour or so and BP comes back to normal for remainder of day. Last trip to cardologist, BP was low. He asked if I had taken the Quinapril that morning which I had. He said don't if BP was low but no instructions for high which is main concern. Any ideas? That high BP in the morning is quite unsettleling.

Hello!

Welcome and thank you for asking on HCM!

I carefully passed through your question and would explain that high blood pressure values (like those that you refer) in the morning indicate that your blood pressure is not under control 24/24 hours and some changes to your actual therapy need to be done.

As the morning blood pressure values are related to the drugs you take in the afternoon (based on the pharmacology of the antihypertensive drugs), an addition of another quinapril tablet would be helpful. This would lead to a better control of blood pressure during the night and early morning.

What is the actual dose of quinapril that you are taking in the morning?

Another option to consider (in case quinapril is counterindicated or you don't tolerate it due to cough, etc.) would be taking a calcium channel blocker like amlodipine in the evening or pm. This change would make the same effect as adding quinapril.

I recommend discussing with your doctor on the above issues.

Hope to have been of help!

Best wishes,

Dr. Iliri
