Question: HI Doc,
I am having tsh of .074 miU/L, t3: .69ng/mL t4: 8.59 microgram/dL, currently taking 100mcg thyronorm daily...should I increase or decrease my medication to increase my tsh? Recently my bp has increased to a high value (around 190)

Brief Answer:
Stick to the dose, focus on hypertension,

Detailed Answer:
Thank you for asking

I read your question and i understand your concern. Your thyroid stimulating hormone is way below normal range is it is 0.074 and not 0.74 ( which is normal value in the range) and your t4 is in normal range. Your T 3 is 69 ng per ml slightly below normal range.

So you are still clinically slight hypothyroid. Now regarding thyronorm one can have upto 1.7 mcg per kg and can take upto 300 mcg in a day if need be. Better to stay in range of 125 mcg per day.

Your blood pressure needs to be brought down first before increasing the dose any further, So i would suggest stick to the dose and focus on the hypertension as that is emergency and hypothyroidism is not.

Talk to your endocrinologist and see what they have to say in this matter.

I hope it helps. Take good care of yourself and dont forget to close the discusison please.

Yes I am concerned about my high bp currently and that is why I put my query here. I have read that a high blood pressure /cholesterol etc is also caused due to thyroid problems. Will increasing the thyronorm medication from 100 too 125 mcg increase or decrease the blood pressure.

Previously I used to take 125 mcg thyronorm, the tsh then went higher than normal, to 8.35 miU/L, when the doc asked the medication to be reduced to 100mcg. Now the TSH is lower at .07...

Right now , my bigger concern is the high bp, and I have read and been told that the thyroid problems contribute in a significant way to this...there has not been much change in my diet, on the contrary I reduced my intake of salt, or any processed food..but the bp seems to be continuing at 190+, so wanted to check with you on this. Will reducing the thyronorm medication reduce the bp?

Brief Answer:
Keep at 100, add BP meds

Detailed Answer:
Thank you for getting back to me

Thyroid hormones induce beta receptors in heart which get over responsive to epinephrines leading to these palpitations and increase blood pressure. That is why thyroid hormones need to be in limits neither more nor less.

The only way to control your blood pressure is to increase the dose of bp medications or even use double or triple anti hypertensives if need be.

Reducing thyroid hormones any further will not be wise. As one needs his thyroid hormones to be in range or else one goes in to other cardiac complications like congestive failure.

Nutshell, stick to the medication dosage and talk to the doctor about increasing BP meds or adding another.

I hope it helps.

Will raising the medication to 112.5 mcg help ? I happened to talk to a telemedicine doctor yesterday, he had suggested reducing my thyronorm medication. That is why I was clarifying on medicine dosage. I do have an appointment scheduled with a primary care physician in the next 10 days, the earliest appointment available

stick to the dose

As I mentioned earlier stick to 100 mcg and no more right now. Cons of increasing the dose out weight the pros. Wait for the appointment. Till then every thing remains the same.

I hope it helps. .

Take care
