Brief Answer:
Thyroid

Detailed Answer:
Sorry to learn about your symptoms.

If you are already on synthroid there is no need to take additional iodine because synthroid contains levothyroxine which is made from iodine.

When you see your PA on Monday, check with her when your next laboratory tests are due.
She will probably order

CBC
CMP
TSH
Free T4
Serum 25 hydroxy Vitamin D
HbA1c

The above tests will give an ideas as to the potential cause of your medical complaints.

Regarding the alkaline water, I generally do not recommend this to anyone but ask your PA if this is beneficial for your kidneys, based on your previous kidney evaluations and opinions form kidney specialist.
It is absolutely not necessary from a thyroid perspective.
