Question: Hi doctor I have benign pacs and the rare pvc according to my tests. Heart is structurally normal and my cardiologist insists that they are caused by my anxiety. He says my heart is very healthy. Is this normal to happen after a stressful day? My last holter recorded 400 pacs over 48 hours always in sinus rhythm. I didn't eat good today and had indigestion. I got up from the couch to carry my daughter upstairs to bed and my heart started racing pretty bad. I think I got more anxious about thinking of my heart racing and it got worse. My heart rate went up to 180 bpm. Is that normal? Can this happen with a panic or anxiety attack of your heart is already racing from going up the stairs?

I read your query and understood your concerns.

What you have experienced is caused by a combination of indigestion, physical activity (standing up immediately and climbing stairs) and anxiety.

It's normal to have tachycardia (up to 200 bpm) and anxiety attacks on this settings And you shouldn't worry about that.

My suggestion is to treat indigestion and to avoid standing up to fast and to do immediately physical activity such as climbing up the stairs. This is the best way to avoid such a findings.

Thank you doctor. Have you seen this in other patients before who have anxiety? Is it common for heart rate to go up like this. I believe it was worse because I had just gone up the stairs carrying my daughter. Also I noticed now that since I have calmed down I could feel some pacs

Hi doctor one last question before I close the discussion. Does it matter how many PACS I have. I know you said if I only have a few it's fine but some days I will only have a few and other nights I will get hundreds or even episodes that last hours long . Especially at night or when I wake up in the morning

Doctor is there a chance these episodes are svt? Maybe the holter didn't pick it up. Also if it is svt would I know the difference? The episode last night lasted 5 to 7 minutes.

There's a very low chance that these episodes are SVT. You should feel the difference between PACs and SVT. It's almost impossible to be missed a SVT on holter monitoring.

I was just wondering if yesterday's episode could have been svt because of the fast heart rate. I didn't feel any pacs until a couple of hours later when I was relaxing

Only tachycardia (fast heart rate) isn't enough to confirm SVT diagnosis. The only way to confirm SVT is ECG or heart monitoring.

based on the symptoms you are describing it looks more like sinus tachycardia (tachycardia with normal rhythm) . However to definitely rule out or in SVT, next time that you will experience tachycardia more than 10 minutes you should go to ER to do an ECG. If normal rhythm than you have had only sinus tachycardia.
