Since how long you had this problem?
How it started first?Do you have any STD in the past?
Did you ever took any treatment for this problem?
What do you exactly mean by sperm coming out at digestion time?
is it the seminal discharge at the time of digestion?
and if so, in how much quantity?
Will wait till I get the above information from you.

My Impression-
You seem to suffer from some organic disease,which is causing this sperm leak at digestion.
Hence it needs to be worked up,which is possible only after more information as asked from above.
