Suggest Treatment For Infant Regurgitation

Question: Our baby girl is 7 weeks and weighs 4.65 kgs. She is completely on formula (Nan pro 1) since her 3rd week. She was 2.75 kg when she was born .

Our problem is that she has become very fussy during feeds since last 10 days. And we feel that she has some problem because of which she is not taking proper feeds even when she is hungry

She was taking proper feeds of 120 ml but since last 10 days whenever we feed her, after taking 60 ml we burp her then when we again try to continue feeding she refuses to take the bottles, screams a lot, becomes very rigid and arches her back. Whereas she seems to be hungry as she sucks her fingers but she doesnt sucks the bottle's nipple. She gets very uncomfortable while burping.

The whole feeding process of taking 90 ml takes 45 min to 1 hour. We feed her every 3-3.5 hours. She never spits out but it seems that she does gets something back in her mouth as she makes expressions of chewing every now and then.

She is very gassy and passes gas during feeds sometimes. Her total feed in a day is approx 550 ml.

I have gone through the details and do understand your concern.
The symptoms which you have mentioned are suggestive of gastroesophageal reflux.
Here the baby gets restless while feeding with regurgitation of feeds and associated with arching back.
I would suggest you to decrease the amount and to increase the frequency of feeds.Burp her well after each feed.Keep her in upright position after feed for few minutes.
I would like to give her esomeprazole granules(an acid reducing agent) along with antiemetic like domstal or motinorm drops.
Please get it prescribed by a pediatrician as these are prescription medicines.

We raised this issue with our paedratician and he prescribed Cypon Drops( cyproheptadine hcl & tricholine citrate drops. Sir, if you can please ler us know what this drops for and is it safe to give it to 7WEEK old baby. Also please let us know how much formula my baby should be taking per day as per her age. Pls let us know the dosage of the medicine suggested by you.

cyproheptadine with tricholine citrate is used as an appetite stimulant to increase the appetite.
I don't prescribe this medication as it has sedative property and should not be given in children.
It is always better to look into the cause rather than to treat symptomatically.
The dose of domperidone for baby of 4.5 kg is about 2 mg,3 times a day (2 ml of syrup and 5 drops of drop)and the dose of esomeprazole is 10 mg/day.
A baby of about 5 kg should consume at least 500 ml of formula per day.
I would suggest you to get it prescribed by a physician.

Thanx for your prompt reply Dr XXXXXXX Just one more query. Shall we also change the formula from Nan Pro to something like Similac Sensitive or Isomeal etc ? or Nan pro would be fine as per you?

Similac sensitive is a low lactose formula and Isomil is lactose free soya based formula.
Similac sensitive is a better choice to start with if needed.
Intolerance to lactose may be a cause of poor feed,fussiness or excess gas formation.
I would suggest you to give the medication as advised and if there is not much improvement of if symptoms recurs you can try changing the formula.Because it would be difficult to assess the underlying cause if we simultaneously change the formula and also add the medication.
