Hello sir, thanks for trusting health care magic.
This is paronychia which is the swelling and infection of nail bed and has formed a lot of pus beneath it so it needs surgical drainage at the earliest.
Don't rake it lightly ad it can damage the nail bed permanently and even cause gangrene of the thumb which will eventually need amputation.
So visit a surgeon at the earliest.
Till then take some good pain killer lje diclofenac along with serratiopeptidase combination three times a day and dip it in hot water 3-4 times a day.
