Brief Answer:
Looks like bedsore, apply topical antibiotic

Detailed Answer:
Hi
Thanks for being at healthcaremagic
I have reviewed the attached photograph and it is looking like a bed sore.
If I was his treating doctor I would recommend him to apply topical antibiotic like Mupirocin or fusidic acid cream on the affected area twice daily for 10-12 days.
Besides this ask him to frequently change his side instead of lying on his back most of the time as this will hasten the healing of this lesion.
Regards
Dr Asmeet
