Question : Hi my boyfriend has a 38 percent motility but everything else is normal and I got a hsg and blood testing done and everything with me is great. We've been trying to get pregnant over a year ... any advice ?

I think you should go for ovulation induction with timed intercourse.

Detailed Answer:
Hi,

I think you can take medicines like clomiphene for the growth of your follicles and track your follicles growth by repeated ultrasound. When follicles reach a size more than 17 to 18 mm, take injection for rupturing the follicles. Be in contact with your boyfriend for next 2 to 3 days after the injection.

Take progesterone for next 2 weeks. Do a urine pregnancy test at home after that. You can try like that for 3 to 6 cycles. The chance to get pregnant in one cycle of ovulation induction with timed intercourse is 7 to 10 percent only. So, try at least 3 to 6 months.

For your boyfriend, he may take some antioxidants and some multivitamin supplements for 3 months. It will increase the motility. Also, it'll be really helpful to quit smoking or drinking (if he does) because these also decreases the motility.

Do a semen culture and sensitivity also because sometimes infections can lead to low motility. He can take Doxycycline antibiotic 2 times per day for 10 days empirically. Please discuss with your doctor regarding all these, and the necessary prescription for these.

If ovulation induction and timed intercourse don't work, then you can go for IUI.

But why would I need to take medication if everything seemed normal?

Hi,

Generally, when a couple tries naturally, there is a 5 to 7 percent chance of getting pregnant in one cycle. So, we recommend to try for at least one year before undergoing any test or treatment. In your case, you told that you have already tried for one year, that's why I suggested you ovulation induction with timed intercourse.

I think if you want to try naturally for few more months, you can do that also. There is no problem in that.

You can track your ovulation by repeated ultrasound or by ovulation detection kits and be in contact with your boyfriend every 2 to 3 days after your periods stop. It will increase chances of your pregnancy. If your follicles are forming on its own and you are ovulating, then you can try for 6 more months without taking any medicines. Just confirm that you are ovulating.
