Suggest Treatment For Injury To Tunica Albugenia

Question: I had an injury to my penis almost 48 hours ago. I had a 100% erection and a lot of force was unintentionally applied to the side of my penis which caused a very sharp intense pain and I quickly lost my erection. Following the incident I felt mild discomfort, and the area was a little red with two small bumps on the side. I took an Ibuprofen after the incident and went to bed. It's been over a day and there is no huge swelling or major discoloration like you would typically see with a penile fracture, but I do have two small semi circumferential bumps in my flaccid penis where the injury occurred and it's slightly red.

I did see an urgent care doctor that told me if I had a fracture I would know and there would be gross busing and swelling "eggplant deformity". No ultrasound or other imaging scans were ordered.

Is it possible to have a small tear tear in my tunica albuginea and have the blood leak into the corpus spongiosum thus leaving none of the typical signs you see with a penile fracture (ie eggplant deformity)?
I have heard about some rare cases people have a penile fracture but without the initial major bruising and swelling. I plan to follow up with a urologist in 2 weeks, but I don't know if I'm doing myself a disservice in waiting so long to get a proper diagnosis.

When I roll the skin over the lumps they do not move. You can see the 2 longitudinal lumps on the right side in the picture below. I did not have these lumps before this incident.

Thanks for your query,based on the facts and photograph that you have posted it appears that you have sustained trivial injury to Tunica Albugenia resulting in to minor bruising and prominence of two tributaries of dorsal vein of penis.

You need to get ultrasound scanning of the penis done at earliest to rule out any major injury .

In the mean while take broad spectrum antibiotic like Cefexime along with anti inflammatory drug like Diclofenac twice daily.

You can apply tight bandage over the penis to prevent further leak.

Do not attempt any sexual activities for couple of days as there is risk of leakage
blood in subcutaneous plane .

Do not worry ,It will take a week or two for complete cure.

When you mention the risk of leaking blood in subcutaneous plane, is this what causes the deformity and swelling in more severe penile fractures or is that caused by bleeding into a different area?

How does one differentiate between a trivial injury to Tunica Albugenia and a major injury? Wouldn't a major one usually be accompanied by major swelling an discoloration? I'm assuming a ultrasound is the only way to completely rule out major injury.

In the incident of tunica tear, emergency surgery seems like the recommended treatment, and the sooner the better in terms of healing and functionality. Is it safe to wait over a week for my appointment (providing the swelling doesn't worsen), or would you recommend I get an emergency ultrasound?

Tunica Albugenia is a sheath that envelop two corporal bodies (Erectile Tissue).Whenever there is major injury (what is called as fracture of the penis)to this sheath blood leaks out from vascular corporeal bodies outside in to subcutaneous plane giving rise to swelling , bruising and deformity of penis of the penis which needs to be treated as a emergency.
However if there is trivial injury in the form of small tear in Tunica Albugenia it gets sealed up with blood clot so one does not get swelling or deformity of the penis .
This is confirmed only on Ultrasound Scanning of Penis.

Since 48 hours have already passed out and there is no swelling and bruising of the penis I can state with confidence that there is nothing to be worried and you can get your Ultrasound Scanning at leisure.

Thanks for the comprehensive answer. That's just the kind of detail I was looking for.

In a case like mine, is a trivial small tear of the Tunica Albugenia ever surgically repaired, or is there no need as long as as it's not leaking into the subcutaneous plane?

As far as I know I don't think I have peyronie's disease, but can a trivial injury such as this trigger the scarring and growth of fibrous plaques? Is there anything I can do during the healing process to prevent this from happening?

Minor injury of Tunica Albugenia once gets sealed up does not require any active intervention.

There are no chances of forming a plaque either.

You have to take care of refraining from sexual activities (Either intercourse or Masturbation) for two weeks .
