Thanks for your query ,based on the facts that you have posted it appears that you have pain in testicle .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you any urinary symptoms like burning ,frequency ,urgency ,and any discharge from penis ?

You need to consult qualified Urologist for clinical examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of Scrotum
3) Blood test for STD.(Considering your wife having affairs the possibility of you acquiring STD has to be ruled out)

You need to take broad spectrum antibiotic like Augmentin along with anti inflammatory drug like Diclofenac twice daily for 10 days

Further treatment will depend upon result of these tests and final diagnosis.

let me explain more, in 1997 i was diagnosed with hyothyroidism my pituitary and thyroid have burned up my wifes first affair was about april of 2010 we got together in sept on december 6 iwas driving and all of the sudden there was a terrible pain in my testicles i had to stop and throw up it seems every time we have sex which is only twice a year because she is gone alot it comes back i have been tested for stds includng hepatitis and have undergone 20 days of doxycyclene there is also a low grade fever that i have all the time since that my doctor cant get rid of at night i always sweat so much i have to sleep on a towel.whatever this is it has something to do with sex with my wife we had sex the first of november and it started 5 weeks later we had sex overchristmas and it started the next day

Thanks for follow up .
Since the symptoms of pain in testicle gets exacerbated after sex it is possible that your wife may have vaginal infection and needs to be confirmed by examination by a qualified Gynecologist .

Your symptoms are mostly due to either infection of Testicle(Orchitis) or Prostate(Prostatitis)

Please get following two basic tests done to confirm the diagnosis
1) Semen culture
2) Ultrasound scanning of scrotum and abdomen
3) Swab test and culture of urethral discharge obtained after prostate massage to rule out Prostatitis.

Orchitis or Prostatitis takes long time to get cures hence one needs to take appropriate antibiotic as per culture report .
