Suggest Treatment For Internal Hemorrhoids And Red Spot On Colons

Question: Hi, my husband has several things going on at once. He has fissures with internal hemmrhoid that won't go back in, the size of half a grape, very little. It has been out for a couple of weeks, maybe three. After colonoscopy and biopsy, it showed a small half dollar size red hot spot on large colons. He's done everything from sitz bath to creams (prepH and Nitroglycerin) to suppository (won't go in) to taking antibiotics (still)! Still the hemorrhoid won't go back in. Because of fissures, it's too painful to push back in. And because of colitis or possible Crohns, doctor doesn't want to cut it. Does it stay this way forever? Could this cause more serious problems if it is not cut? His rectal surgeon now wants him to see a GI. As his wife, I'm doing all I can to support, prayer, cook healthier foods and crying (not so good, but I don't want to lose my husband because of something we are missing here)! Please, what can we do to get this back in. He has no other symptoms other than yellow orange discharge from or around the tiny hemmrhoid. He feels he's be better if it would go back in. It's painful to sit and BM is all. God help me, please, what are your recommendations at this point. He's tired of bouncing around from doc to doc and wants to get back to work. Missed weeks without pay...I do not work due to my own disability. His job is beginning to question him, postal worker. Please help? Anything! That we've not yet tried? Thank you and God bless!

Hi my dear,

I am sorry for the situation your husband is going through.

Pain is often a fastidious symptom of hemorrhoids and anal fissure.

This is because the anal mucosa is torn/damaged. He might have been suffering of severe constipation which has been caused him to strain at the extend of causing anal fissure.

This hemorrhoid is small in size, and does not explain the pain he is having.

If the surgeon did not operate, it means that he does not need it.

Further investigations are advised to rule out other inflammatory bowel disease including colonoscopy, and blood works.

For the moment, the very first thing is to prevent constipation and excessive straining.

encourage him to :
- increase water intake
- have more fiber rich foods like prunes, pear, figs, etc.
- fiber supplements containing psyllium can help
- take laxatives and stool softeners like lactulose

until you see GI specialist.

This is a temporary situation although painful.

Thank you so much! In prayer that this is a temporary situation. He was under the care of a rectal surgeon, but he's referring him to a GI now to look further into the colonoscopy results he did have that indicated from a biopsy taken that he foes have colitis. He described it as being red hot and on fire! But he has no stomach pain, dizziness or anything. Its just the hemmrhoid that causes pain and spasms were worse but better today. Attached are pics from two days ago. Was concerned about the discharge. Before and after i cleaned it. And I do not know what fissures looks like but he feels them.

After seeing the photos that you have uploaded I would like to say that:
1. Your husband has external hemorrhoids that explain the anal discomfort

2. I can't see anal fissure but since he has discharged this is a sing of anal sphincter damage; probably form anal fissure.

3.With regards to half dollar size red hot spot on large colon may be rectal solitary ulcer/M. Crohn. Discus with your GI about cortisone use.
If I have Colonoscopy report I would give an accurate opinion to this finding.

For the moment keep going with preparation H and avoid constipation.

Oh my goodness bless you! Ok! This gives me some relief. Im trying so hard to be supportive and gather any information i can to make this an easier journey for him, myself and our son. Im going to try to attach the report for you. And the biopsy report reads Tissue sent: descending colon biopsy, screen inflamed colon, final pathological diagnosis: focal active colitis. Note: The morphologic features are nonspecific. Acute self-limited (infectious) colitis and ischemic colitis amd diagnostic considerations. Gross description: Specimen labeled descending colon biopsy": One tissue piece, in formalin, 0.3 x 0.3 x 0.3 cm, filtered and entirely submitted in A1.

My husband and I have no clue what this all means after leaving the doctors office. Hes terrified and myself trying tl be strong in the Lord and remain in prayer.

So theres a chance the hemrhoid can go bavk in on its on even after almost 3weeks? He said its his only discomfort with some discharge of course.

At this point his doctor is referring him to someone else. And that is why we are concerned. He has had so many creams to include nitro, anal pram and prepH, Tucks and sitz bath. He's thinking of retiring from his job now. Should he hold off a lottle longer, give it some time? We just dont know what to do.

Focal active colitis are inflammatory changes of colon mucosa and is suggestive for acute self limited/infectious colitis (acute bacterial infection of the colon.
It also may include some cases of treated ulcerative colitis, Crohn colitis, ischemia, NSAID related colitis.

By the definition it's clear that this isn't a disease. It's only an endoscopic/biopsy finding and is mostly related to acute bacterial infection.

Your husband hemorrhoids will be healed with proper treatment. He should have more patience because everything it's going to be normal again.

He is extremely relieved! Bless you! We are very happy with your advice and explanations of his reports. We have one other question. Can this be sexually transmitted? Like can this become proctitis that is caused by colitis or Crohns? Thats our only other concern.

Now let me answer to your queries.
1.     The endoscopic findings (especially focal active colitis) aren’t sexually transmitted and you shouldn’t worry about that.
2.     Only in rare cases focal active colitis may be an early sign for Chrohn disease or ulcerative colitis. This is why it’s important to continue with regularly follow ups with your gastroenterologist (every one year).
