Suggest Treatment For Joints Pain

Question: I am a RA patient since 2006 and having regular medicine as below since then :
1. SAZO1000 ( 2 Tables every day - 1 morning / 1 evening )
2. Folvite 5mg ( once in a week )
3. Folitrax 15mg ( once in a week ) -----Doctor prescribed in Dec recently when my pain & ESR/RCP values gone high.
4. T.E Cort 6mg ( One daily ) --- -----Doctor prescribed me recently when my pain & ESR/RCP vales gone high----- But stopped this medicine after I had fever for 10 days with out any malaria/dengue virus attack etc .

Now, again since last 10 days I am suffering for high pains in all joints and not in a good situation and below is the medicine doctor prescribed me today to have for one month and need to show him again after that . Request you to give your expert opinion if below medicines are ok & safe to continue .

1. Injection Folitrax - 1ml doze ( Once in a week ) --use insulin injection
2. Sazo 1000mg ---- Continue as it is
3. Folvite 5mg --- Continue as it is
4. Proxym XT --- 2 Tablets one in morning /one in evening for next 2 weeks only and then one daily
5. Wysolone 10mg - 1 tablet in morning everyday
6. HCQS 400MG - 1 tablet in evening everyday

Please find the recent medical test reports values which I have done yesterday in Golwilkar Labs :
ESR 59 , CRP 50.66 , TLC 6900 , PLT 2.35 , SGPT 28 , URINE PC 1-2 , TSH 4.78

Thank you for posting your query on HCM.

Well I have gone through your entire history and I think your doctor is treating you appropriately .But your rheumatoid activity is quite high despite being on 3 drugs and use of steroids so its prudent to consider biological drugs like Rituximab .

Using steroids to control the disease activity is a usual practise and helpful to patients so you need not worry about it .

Side effects of these drugs can be many but generally bone marrow depression and liver function derangement is quite common which are generally monitored by regular blood testing .

Any other concerns please ask .

I would like to know how Rituximab drug will help me and due you suggest to have this drug along with other medicines as below.

3. Folvite 5mg --- Continue as it is ( folic acid ---- hope no side effect )

4. Proxym XT --- 2 Tablets one in morning /one in evening for next 2 weeks only and then one daily ( is this steroid and any major side effect risk )

5. Wysolone 10mg - 1 tablet in morning everyday
( is this steroid and any major side effect risk )

6. HCQS 400MG - 1 tablet in evening everyday
( is this steroid and any major side effect risk )

Please suggest so that I can continue above drugs. Also, let me know the important tests which I should keep doing in regular period.

Also, if I get fine in a months time what all above medicines need to be stopped immediately.

Dear Sir,

Thank you for reverting back.

Well in the medicines that you are taking only wysolone is steroid rest others are disease modifying agents . And it needs to be tapered as soon as the disease comes under control .

Rituximab is a biological drug that is often used in patients that do not have s good response even after optimal oral therapy but even with that you will have to continue medicines but their dosage will be reduced and may be we can hold the disease on lesser number of drugs .

On these drugs it is important to get CBC, LFT, RFT done every 3 months atleast .Also while taking steroids calcium and vitamin D supplementation is also recommended as it weakens the bone . Also avoid direct sunlight as HCQS may lead to pigmentation of skin .

Dr. XXXXXXX thanks for your inputs. I am sorry to ask this question to you but thought to clear my doubts. Please confirm if you can advise me for any Ayurvedic / homeopathic medication for this disease. As, I see Allopathic medicines will have side effects and surely with my 67age all these medicine's to continue will be tough for me. Request you to help me with your honest feedback and suggestions.

Dear Sir ,

Since am not an expert for alternative medicine I will reserve my views on this topic . But my personal advise would be to go with allopathy only because we should be afraid of the disease not the treatment . All these drugs are time tested and have a large data to support their use and you will be monitored regularly for side effects so dont worry . Rest is your personal choice .
