Suggest Treatment For Kidney Damage

Question: in 87 YEAR OLD FEMALE WHO WAS TAKING DOLO 650 BD ( paracetomol) and ultracet FOR BACK PAIN DUE TO LUMBAR PROBLEMS. SHE HAD ACUTE RENAL FALIURE, CREATININE WENT TO 8 , urea 240 ,COULD THIS BE REVESED , after lasix creatinine reduce to 7 . whatshould be mode of tratment, doctors are aagainst dialysis due to her age, and they say kidney is 1005 damaged so dialysis will be no good.
if its becuse of toxicity of dolo or ultracet etc then can it be reversed and how kindly suggest URGENTLY

I am Dr. Prakash HM and I will be answering your query.

Looking at the query, my opinion is as follows:

1. If its due to dolo or ultracet, renal damage cannot be reversed. However, the condition can be improved almost completely functionally (She has two functioning kidneys). Damage, once it has occurred, cannot be reversed. Renal failure at this dose of dolo or ultracet is rare.

2. I do not expect this dose, unless overdosed by atleast 10 times or more to develop any acute renal failure. Usually liver failure is more common and renal failure, without liver failure with this drugs overdose is usually rare.

3. Dialysis at this age is risky due to fluid imbalance. This could make the condition more critical.

4. Lasix is at presently helping improving the condition. Treatment needs to be continued. She will recover. After this acute condition, depending on damage to kidney, she might go for chronic renal failure.

Not to worry, she will recover. It will take longer because of her age. Her other chronic disorders like hypertension or diabetes mellitus will further slow down, but she will recover in due time.

sorry that i did not informed other details, she had some infection also, as her neutrophil count is above 90 % lymphocyte at are only 5% plateletes 80,000.
before we took her to hospital she had high grade fever 104 which was not coming down , some antibiotics were given.
now the antibiotics being given in hospital are as follows
metrogyl, lesuride, sublactam + cefperazone, timidazoal IV,linizeolide IV ceftriaxone IP Pan IV thyronorm , lasix.
urea yesterdaywas 250 creatinine 6.8 . pulse 66, oxygen saturation is manintained at 95-96 ,withh oxygen its 100, there is edema in hands and legs and now a bit on face. but urine outputis there 500-600 ml.Hb is 7.8
so renal faliure could be subsequent to infection also , combined with dolo etc.

Hi,
Thanks for asking again.

Based on further details, I can comment as follows:

1. Infection leading on to sepsis is causing renal failure here.

2. All the vital functions and oxygen saturation is good. Urine output of 600 ml with diuretics is good and it will improve.

3. She is on multiple antibiotics and it appears, she is responding well to treatment looking at the vital functions.

4. Low hemoglobin has contributed to mild cardiac failure which has led onto edema. Due to renal failure, edema of face is starting. As the situation improves, overall edema will reduce. She might require a packed red cell transfusion, once she recovers.

5. Continue with the same treatment. No dialysis necessary.

6. Present renal failure is due to infection only. The amount of dolo etc, being taken would not usually cause any damage.

7. High neutrophil counts suggest bacterial infection and will come down as the treatment starts taking effect.
