Suggest Treatment For Kidney Stones And Pain In Side Of Abdomen

Question: Hello Doctor,
Sir I m having kidney stone 5mm in left and 17mm in right...inflammed and gaseous distended bowel loops are seen at both iliac fossa and umbilical region....hepatomegaly with fatty infiltration grade-2....bilateral nephrolethiasis...plz suggest me cure as I m having severe pain on right side of abdomen.

Please find my observations below.

1. There is a large 17 mm stone in the right kidney. This can cause obstruction to the passage of urine and must be treated immediately to relieve your pain and prevent permanent damage to your right kidney. Please consult the urologist immediately, within a day or two and discuss the treatment for the stone. Most likely a lithotripsy procedure might be planned and this will be done as a minor surgical procedure. If this stone is not treated within 15 days then there are chances of irreversible kidney damage due to urinary outflow obstruction.

2. The left kidney stone is measuring 5 mm and is small. This can be treated by conservative means for the present. Follow up ultrasounds will be needed as you have a tendency of developing kidney stones. Please be on close follow up with the urologist.

3. Hepatomegaly and grade 2 fatty infiltration might be due to increased intake of fatty foods and alcohol. Please review your diet and change over to a healthy low oil diet plan. Also get checked for lipid profile and this tells if you have high cholesterol.

4. The severe right sided pain is due to the stone in right kidney. Please drink adequate amounts of water and visit washrooms regularly. You need to take the following precautions to prevent formation of more stones in future.

DOs for kidney stones
(i) Drinking as much as 1.5 to 2.5 liters a day may help flush out your urinary system. This will also allow small stones to pass through.
(ii) Visit washrooms regularly and empty your bladder when you feel like doing urination.
(iii) Please take fruits like apple, papaya, watermelon, pineapple, sweetlime, bananas and orange, vegetables like, white gourd, ladyfinger, pumpkin, pulses and legumes.
(v) If you have sharp pain then you should consult your doctor and take pain reliever medications under strict supervision.

DONTs for kidney stones
(i) Please avoid holding urine for long intervals.
(ii) Please reduce foods containing oxalates like non veg, vegetables like spinach, tomato, cauliflower, brinjals, nuts like cashew, pista and almonds.
(iii) Decrease extra salt in foods.
(iv) Avoid spicy pickles.
(v) Please eat meat products in normal amounts, avoid eating too much.

1. Stones can be due to many minerals like oxalates, uric acid and in many cases patients have mixed stones. Right now the focus should be on treating your condition and not thinking on why the stones formed in the kidneys.

2. While most of the tests are better to be done after consulting the urologist, you can get basic kidney function test like serum creatinine and blood urea done. This will tell if the stones have affected your kidneys in any way. A large stone can cause significant effects on the kidney if not treated on time.

Sir, I have gastritis...my uric acid level is 4.5 and creatinine is 1.4...should I go for any other test...motion is little bit loose...sometimes feels like thorns pricking in lower abdomen...why sir???

1. Your uric acid is normal and serum creatinine is at upper limit of normal. This suggests that you should consult the urologist immediately and take treatment for kidney stones as discussed above.

2. Urine and motion are generated by different processes. It is advised that if you are having gastritis then please avoid spicy and fatty foods. Please have small meals at regular intervals. Do not skip meals.

3. Thorns pricking in lower abdomen can be due to stones in the kidneys. This pain is felt along the sides and radiate to lower abdomen. There can be small stones that might cause pain in pelvic region other that the stones seen on ultrasound scan.

4. For loose motion please see that the food and water that you take is clean and hygienic
