Brief Answer:
multiple possibilities.

Detailed Answer:
Hello and welcome,

I understand the issue.

There are multiple possibilities with such lump. The commonest ones are infection leading to cyst formation and then they develop multiple cavities.

Lipomas or tumors of fat tissue are also common. Fibromas, hemangiomas and nerve cell tumros may also occur here.

Other conditions are viral warts and rarely cancers.

For similar women in my clinic, I prefer an examination first as it gives an insight into possible causes.
As the lesions are painful, infection is always a possibility and so I prefer to give a course of antibiotics taken orally as well as locally and observe for 7 days. If it still persists then a biopsy is a good option.
Even if your doctor suspects a lipoma or fat tissue tumor, it is advisable to get a biposy done so that a proper diagnosis can be established. Sometimes it is possible to do an excisional biopsy where we remove the entire tumor and send it for microscopic examination.

So do not worry. Just discuss the options with your doctor and then after a proper diagnosis treatment can be initiated.
