Brief Answer:
Combination of medicines for chemotherapy will help.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
I have gone through all the reports. Leiomyosarcome of the lung with involvement of mediastenal lymph nodes is difficult to treat. It is best to restrict the further spread of the problem by chemotherapy. Radiation therapy is not likely to help him much.
It is better to start chemotherapy with a combination of agents rather than cisplatin alone to contain the tumor. A few cycles of chemotherapy will help in stopping the further progression.
He needs a regular follow up Ct scan chest every 8-10 weeks to monitor the progress.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.
Regards.

Brief Answer:
Surgery not recommended right now.

Detailed Answer:
Hello.
Thanks for writing again.
Surgery is not recommended with mediatsenal lymph node involvement. It is best to restrict the growth of tumor with chemotherapy first and after that surgery can be considered.
Combination of Cisplatin with Doxorubicin will be better.
Hope my answer is helpful.
