Brief Answer:
Need to rule out cardiac issues first

Detailed Answer:
Hello Sir.

As per your symptoms, the possibility of autonomic dysfunction is high.

At this point since you have an elevated blood pressure, a complete cardiac evaluation is mandatory.

For this, advise you to take a 24 Holter ECG and a 2D echo of your heart.

Based on your test results, further management can be planned out.

Thank you.
