I had endoscopy that came back normal awhile ago. im not dehydrated as I drink a lot of water. im scared about waking up in the middle of the night feeling light headed as if ive been drowning underwater all night. I wake up2-3 times a night and always feel tired throughout the day. what could this be?

This kind of sudden onset awakening, lightheadedness, breathlessness in middle of the night are seen in heart failure.

I had echo in January it was normal had ekg chest x-ray several over last few months and a calcium CT score of zero I also have good blood pressure

I should also mention that I am being treated for panic disorder and general anxiety disorder. I also have had several complete blood count all normal. Diabetes well I had a high reading of 180 doctor said it was due to elevated levels of cortisol in my blood. But I only have this breathless problem in the middle of the night during the day I go for some walks and I walk up and down stairs fairly easy. I do get panic symptoms when I try to exercise though I don't think I have heart disease I am bring tested for ehlers Sarkis syndrome.( Not sure if that's relevant )

So, your all reports are normal. So no need to worry for major heart or lung related diseases.
Since you are also suffering from anxiety and panic disorder also we should rule out psychiatric cause for your symptoms.
So please let me know
1. Are you having stressful life?
2. Are you consulting psychiatrist for this?

It is mostly worsening of panic disorder.

Thanks for your follow up question on Healthcare Magic.
I can understand your concern.
In my opinion, you are mostly having worsening of panic disorder which is causing all these symptoms.
So better to consult your psychiatrist and discuss all these.
You might need addition of new drug or increase the dose of current drug.
No need to worry for major illness.

Detailed Answer:
Thanks for your follow up question on Healthcare Magic.
I can understand your concern.
I agree that these symptoms might be due to side effect of psychiatric drugs.
But for me, these are more suggestive of worsening of panic disorder.
Sudden onset breathlessness, midnight awakening, bad dreams etc are symptoms of panic attacks.
So better to consult your psychiatrist and discuss all these symptoms and find out what psychiatrist has to say only these symptoms.
Hope I have solved your query.
