Hello,

Thank you for posting your query and the picture.

From the image you have attached, I strongly suspect a localised allergic reaction (localised erythematous rash or you can call it hives).

It could be due to contact with some sort of chemical, or it could be due to an insect bite or just exposure to sunlight for prolonged periods.

The diagnosis depends on the exact history and physical examination.

It would help me if you can describe how you got this rash..

How long you have had this rash, and does it itch, or burn or do you have any pain in the area..

If you can provide all the details, even if you feel the symptoms to be insignificant, if you can share them, I would be able to get a better idea about what the exact cause is.

For the time being, I would suggest you to take Benadryl tablets, or you can take Tab Ebastine 10 mg twice a day for three days and see how it goes.

Apply an emollient to the area to keep you from scratching it repeatedly.

However, if you develop boils or painful swellings in the region, or if the entire limb swells up and becomes painful and if you develop fever, I would urge you to go to the ER and get yourself evaluated, as that might indicate serious infection.

I have reviewed the attached photograph carefully again and I don't see any dark spots.

It maybe because the dark spots/mites were not present in the area you have photographed when you took the pic.

It is quite difficult to visualize such problems on a photograph.

If you are sure about dark moving things under the skin, you must consult a dermatologist as soon as possible.

They might be ticks or mites that burrow under the skin and they can cause several diseases, however, if this is going on for many months or years, then it would rule out such diseases.

Please do consult your dermatologist (Skin specialist) at the earliest and get yourself examined to find out the exact cause.

If it is due to ticks or mites, then the treatment would be easy, if it is due to autoimmune disorders then it may be a bit more tedious to treat.

Most important thing to do is to correctly diagnose the cause.

Autoimmune disorder/disease are a group of diseases wherein your body produces antibodies against your own body cells. You might have heard of disease called lupus or SLE, that is an example of autoimmune diseases.

Autoimmune disease is not the same as Hepatitis C, Hepatitis C is caused by infection with Hepatitis C virus, and it will not have the features like you are having.

I mentioned autoimmune diseases because this problem of yours has been going on for quite some time now, and only autoimmune disease cause such chronic waxing and waning symptoms.
