Suggest Treatment For Low Hemoglobin Count And High ESR Levels

Question: hi my name is XXXXXXX
this is for my sister she is 27 , in her blood check up its shown as haemoglobin count is very low 5.6g/di and ESR is also high 80mm/hr and lymphocytes 51%
so please suggest good treatment and tell how much risk in this and how to get rid of this stage

I am Dr. Jyothi BL and I will be answering your query.

1. Hemoglobin is moderately low. Complete hemogram, Vit B12 assay, iron studies, reticulocyte count, stool examination, etc is necessary to identify the type of anemia.

2. So low hemoglobin, in females is usually nutritional. She needs to improve her diet with Vit B12, Folic acid and iron (Fish, lean meat, legumes, pulses, nuts, green leafy vegetables, eggs etc). Also until it reaches 10g%, she should take B-complex and iron tablets daily or iron injections. Discuss all about this with your doctor.

3. Possible worm infestation in intestines can lead to malnutrition. She can take Albendazole for it.

4. Lymphocytosis is only mild, not to be worried about at present.

5. ESR is elevated due to anemia. Once anemia corrects, it will come back to normal.

6. No blood transfusion necessary for this amount of hemoglobin, if she does not have any swelling of feet or breathing difficulty. Ask her to avoid heavy exercises.

7. She should improve her overall diet. She appears to be severely malnourished, based on blood reports.

right now we consult one doctor , he also told same about worm infection, so he give needed medicine one tab she take now and the another for next week. and he told to wait till 2 weeks and to take a blood checkup . so is that safe to wait till this time. and she didnt have problem with swelling of feet or breathing difficulty.
if she bent down and after that she feeling little miss balance type for few seconds. so please suggest , what we should do next ,or is that safe to wait for 2 weeks for the next blood checkup ? and please suggest what all precautions we have to take . and please tell how much risk and what all possibilities to get rid off this

Thanks for asking again.

Yes, its safe to wait for 2 weeks for the test. Even after 2 weeks, maybe minimal improvement of 0.5g% maybe seen with good nutrition and supplements. Monthly 1-2g% improvement will be seen usually.

You can take the tablets as suggested by your doctor for worm infestation.

She is having postural hypotension, she can take more fluids to reduce balance problems. Ask her to avoid rapid postural change eg: from sitting to standing or sleeping to sitting, etc.

Improvement in diet as suggested earlier along with nutritional supplements is adequate.

No specific precautions necessary than avoiding excessive physical exertion until hemoglobin comes to 10g%.

Nothing else to be worried.
