Suggest Treatment For Lower Back Muscle Pain

Tylenol is not working well.
Proper review, treatment depends on cause
Detailed Answer:
Hi and thanks for the query,
It might be important to note that treatment of lower back muscle pain might depend on the cause of the pain. It might be important therefore to check for causes like trauma, inflammation , lack of minerals like Calcium and Magnesium before initiating treatment.
NSAIDS are not suited in your case, as you do mention. However, drugs like morphines, Tramadol, caffeine containing drugs, muscle relaxants like decontractyl could be useful. Physiotherapy does help in some circumstances. Ice packs can be used if pain is so severe.
Doing a complete electrolyte panel to check for any ionic imbalance, X rays for the spine, ultrasounds and CT scans, blood tests like the erythrocyte sedimentation rate (ESR) to check for levels of inflammation could help in identification of possible drug adapted to the cause.
The opinion of your primary care doctor would be most appreciated. Kind regards as I wish you the best of health. Please, feel free asking further questions in case of any clarifications or concerns.
Kind regards
Dr Bain

CT scan showed good. Muscle was inflamed a little directly above my tail bone.
He said this is a common problem and don't rest no more then I have to. And do not lay down except to sleep at night as this will not help either.
So, Basically walk and he gave me some stretching exercises.
CT scan showed no sprains. Which is good.
Lumbar spine X ray showed no bone or disc damage.
Complete blood test showed my electrolytes were fine.
But the blood test showed I am 77% low in vitamin D
74% low in calcium.
25% low in magnesium.
So therefore my regular physician suggestion is to take the following.
Magnesium 400mg three times daily.
Calcium 600 mg twice daily.
Vitamin D 400 IU once daily.
And also add.
Fish oil 1000mg with omega 3 - 300mg three times daily.u
And take 500mg cinnamon to prevent nausea.
Does this vitamin regiment sound good to you? And is there anything further you believe I should do?
Thank You, XXXXXXX
Treatment plan is satisfactory in my opinion
Detailed Answer:
Hi and thanks for the update,
I strongly believe the prescription is ok. To add, I would suggest a local Diclofenac creme to be applied/massaged around the pain area to target the mild inflammation.
The drugs prescribed in my opinion are Ok. I suggest you take the drugs as prescribed as I hope all goes well. Wishing you good health.

I will go back and ask my regular physician if he will write a prescription.
In case he doesn't.
Would you please give me your opinion on the best over the counter substitute?
I have tried icy hot, XXXXXXX XXXXXXX and cold packs.
Any suggestions would be much appreciated.
Thanks Dr Bain
No worries, get drugs first.
Detailed Answer:
Hi and thanks for the query,
I do perfectly understand that with treatment options you have tried, you might have to worry on the potential of these prescribed drugs either working or not.
However, I will be more optimistic to think that this treatment options would give you remarkable relief. I would reserve any further prescriptions, and suggest you just get your General practitioner to add Diclofenac oral and creams for massage for you. I suggest you just be patient and get the drugs. Coupled with the former treatment regimen, I am positive that you shall feel better.
Kind regards as I wish you the best of health.
