Suggest Treatment For Lump And Dark Pigmentation On The Lower Legs

Question: I hit my lower leg badly 7 weeks ago and I am still left with lump that goes dark in colour like a bruise ( a bit lighter in the morning after sleeping). My GP told me to massage it - this is not working. Will this ever go away or should I go to a vein clinic at this stage.

Thank you for posting the query.

From the history,it seems that the lump that you are talking about is a clotted haematoma.

After any severe injury, the underlying blood vessels and tiny capillaries are injured,causing the blood to accumulate at the injured site.Ideally,this clot should resolve naturally and completely go away in about 2-3 weeks time.

In your case,the resolution process seems to be delayed. The clot may have become semi-solid in consistency (almost thick gel-like).

Such haematomas are hard to resolve once 4 weeks have passed by,post-injury.

You should take fibrinolytic and anti-inflammatory tablets,apply compression bandage,keep the leg elevated over a pillow during night.

Assess the improvement after 5 days. If it shows signs of improvement,you can continue with the same treatment.

If it shows no signs of resolution,then you should consult a surgeon and get the haematoma drained surgically.

IMPORTANT - DO NOT APPLY ANYTHING WARM / HOT OVER THE LUMP. IT MAY CAUSE ABSCESS FORMATION.

Can you give me an example of a fibrinolytic drug? Can I get it in a pharmacy or is it prescription only? Re anti inflammatory- would that be ibuprofen?
A secondary concern is I will be starting Ivf in a week or so and I will be on 5 x fematab a day and then clexane to begin with to thin and prevent clots from the fematab . Does the injury on my leg carry any increased risk of clots forming in that area?

Hi,

Fibrinolytic drugs are the medicines that contain either of the following-
Trypsin, chymotrypsin, bromelain, rutoside, serratiopeptidase, etc. Most of these are available on a prescription.

Yes, ibuprofen is antiinflammatory drug.

Clexane won't cause much of a risk to the leg injury haematoma.

Prothrombin time,Bleeding time ,clotting time should be monitored at regular intervals once you start on clexane to assess the extent of blood thinning.

Do not forget to inform your doctor about the leg injury before he starts clexane.
