If there is frequent loose stool associated with cough, this may be due to some viral infection.
Here symptomatic cough management with some antiallergic and bronchodilator syrup along with fluid should be given to avoid dehydration.
Sorry for inconvenience but please answer my queries,so that I can guide you further.
Regards

Her cough is quite bad and has had it for 3 to 4 days the lump below her ankle is quite squishy and when pushed down returns back up loose stooles and rsther sleepy but can stay awake
The past couple of hours she has felt quite thirsty she is diabetic but all her blood sugars are ok
The lump doesnt really hurt but must have appeared quite recently as havent seen this before
Give plenty of fluid, get her evaluated by a physician

Thanks for follow up
Frequent loose stool can cause lethargy and also due to the same child is thirsty.
I would suggest you to give plenty of fluid (ORS) to prevent dehydration and any electrolyte imbalance.
Give some cough syrup (with antiallergic like chlorpheniramine, decongestant like phenylephrine) along with bronchodilator like salbutamol or terbutaline to relieve cough.
You can also give steam inhalation.
The swelling below ankle can be due to early cellulitis (infection). If possible attach an image so that I can help you out.
I won't get overly worried if child is passing urine adequately, accepting orally well, there is no breathing problem and her blood sugar is normal.
I would suggest you to get her clinically examined by a physician so that he can assess the severity of condition and accordingly investigation and management.
I shall be glad to answer if you have any more query.
Regards

Thanks for the idea
She has had asthma in the past but she got cleared years ago
Get her clinically examined by your physician

Thanks for the feedback
Although to me it is difficult to diagnose; after going through the image and reviewing the character of lump which you have decribed I think it may be early cellulitis.
Please get it evaluated by a physician for proper diagnosis and management accordingly.
