Suggest Treatment For Lung Infection

Question: Good afternoon , Doctor
Sorry to bother you , the patient had status post left lobectomy when he was 20). He is 68 years old now .He has just changing his current medication to Exforge for controlling his blood pressure . Recently he caught a flu and needs some advice with his yellow mucus and coughing . Does klacid work better with lung infection compared with Augmentin duo forte 125mg875mg?? Or what are the differences between this two medications ?? The patient is on Exforge at the moment , is it ok to take antibodic with it ??

Hello dear, thanks for your question on HCM.
I can understand your situation and problem.
Ideal way to start antibiotic is by reviewing culture and sensitivity report.
So in my opinion, you should first get done sputum culture to find out the causative organism and then sensitivity report to start appropriate antibiotic therapy.
Klacid is clarithromycin which is macrolide group of antibiotic. It is effective against gram positive , few gram negative and atypical bacteria.
Augmenting forte is having amoxicillin+clavulinic acid. It is active mainly against gram positive and gram negative bacteria.
So klacid has more advantage over augmenting as it is covering atypical bacteria too.
But I still insist to get done sputum culture and sensitivity to identify the causative organism and guiding appropriate antibiotic.
And both drug are safe with exforge. So no need to worry much for drug interaction.

Thank you for the reply . He is on klacid 250 mg the sixth day and finds the mucus still yellow with coughing . Do you suggest him to have longer than 7 days or try 500 mg twice a day for another course. Or he should try Augmentin duo forte 125mg875mg?? He is currently traveling in Asia ! Thank you

He is having yellowish sputum even after 7days of clarithromycin. This suggest that the antibiotic is not working.
I still insist for sputum culture and sensitivity to identify the causative organism and starting appropriate antibiotic.
Inadvertent use of antibiotics will lead to delayed recovery and increase drug resistance.

Dear doctor , I really understand your suggestion but it happens when he is overseas now so I do need suggestion for him to take now . Increase to klacid 500 mg twice a day or taking Augmentin duo forte 125mg875mg. Thank you .
Also , please let me know where we can get sputum culture to find out the causative organism and then sensitivity report . Your help is much appreciated !!

In my opinion you should start augmentin duo for him.
I want to ask few more things.
1. Is he frequent traveller in Asia region?
2. Did he taken BCG vaccine (for tuberculosis)?
2. Did he taken pneumpcoccal and influenza vaccine?
Sputum culture and sensitivity are done in laboratory where microbiology department is functional. My suggestion for this test is to get done in good accredited laboratory.
Please reply me answers of above questions,so that I can guide you better.

Thank you for the reply .
He does travel to Asia every second month and he did have flu vaccine every year . For the test , do you have any of good labs you can recommend in melbourne ?? Thank you again for the help and I will ask him to start Augmentin duo forte 125mg875mg right away .

Thanks for your query on HCM. XXXXXXX laboratories are good for microbiology in Melbourne.
I also advice him to vaccinate with BCG and pneumpcoccal to protect himself against tuberculosis and pneumpcoccal pneumonia respectively. Incidence of both of these are high in people travelling to Asia.
