Suggest Treatment For Lupus

Question: Hello,

I am a 45 y/o woman with lupus, joint mobility syndrome, and pulmonary problems. I have recently been hospitalized, again, for ischemia on left side of chest. My bp has been steadily getting higher. I just noticed a 3-4 inch deep indentation on left lower leg. I had surgery on same leg for joint PBS about 4 yrs ago. I've had chronic pain, numbness and tingling since. After several bouts w bronchial pneumonia and a mva(1994), resulting in two pneumo thoraxes, I'm concerned that my pain in chest that occurs weekly for past 5 is pulmonary, or it's actually a cardio issue. I'm absolutely baffled by the leg indentation and shooting pain radiating around the site.Please advise as I'm an expat in Malang, Indonesia and needless to say, the healthcare isn't quite trustworthy. I took a 50 mg tramadol over 6 hrs ago and it hasn't helped w the throbbing or shooting pains.

lupus is multisystem disease.

Hi,

Thanks for your question on HCM.

Lupus is having multisystem involvement and the course of the disease is not stable over the period of time. There are ups and downs in the disease activity.

So whenever there is exacerbation of lupus you need to take oral steroids in tapering dose and along with this you need to take low dose immunosuppressive therapy either azathioprine or methotrexate.
So you have to control the inflammation first. I advice you to consult rheumatologist and discuss these things. About your current issues I think you should get done
1. Chest x ray to rule out pneumothorax.
2. CT Pulmonary Angio to rule out pulmonary infarction.
3. Bilateral lower limb arterial doppler to rule out lower limb ischemic ulcers.

Lupus can affect vessels of the body and cause thromboembolism and thus ischemia. So your chest pain can be due to either pulmonary infarction or pneumothorax. And leg indentation must be due to chronic ischemic leg ulcers.
