Question : it started last year. there were two on his arm at the begginning. Red, firm and bigger than it looks under the skin. it comes and goes slowly usually. But now it gets bigger and morr red while he's been dry coughing for more than a month. Lymph nodes swollen sometimes

Brief Answer:
Looks like eczematous dermatitis, go for FNAC

Detailed Answer:
I have reviewed the attached photographs and it looks like an eczematous dermatitis.
If I was your treating doctor I would recommend you to apply topical steroid like cortisone cream on the affected areas twice daily along with application of a moisturising lotion on the entire arms liberally twice daily.
Enlargement of lymph nodes seems to be unrelated to thid.
For enlarged lymph nodes you can go for fine needle aspiration cytology test of the lymph nodes to find out what is the cause of their enlargement.
