Brief Answer:
probably major depression, appropriate drugs given.

Detailed Answer:
Hello
Welcome to HCM.

I have read your query and understand your concerns completely.

You are feeling depressed, lack of interest, not working, eating or sleeping, negative thoughts and aloofness and suicidal ideas. You are having these symptoms since last 1 year.

Your description is highly suggestive that you are suffering from major depression.

You have been prescribed zicam and feliz-s (escitalopram).

I think you have been prescribed right drugs.
Escitalopram is a good antidepressant. This should improve your symptoms.
If there is no improvement in 2 weeks of taking the drug, then you should request your doctor for change of drug or dose increment.

Other than this,
1. You should continue with Art of living course.
2. Eat green vegetables and fruits. These are natural mood elevators.
3. Omega-3 fatty acid supplements can also help in improving your symptoms.
4. You can read some motivational books or stories to motivate yourself.
5. Its good that you have quit smoking. Do not turn to any other drug abuse.
6. Share your problem with you family and friends. Its good to vent out your feelings in front of someone.

Married life will improve once your depressive symptoms will improve.

Hope this helps.

Feel free to discuss further if you have more doubts.

I wish you well.

Take care.
Regards.

Dear Doctor,
Thank you very much for your reply and appreciate your gesture.

As written, I would also like to highlight that there is no one to listen to me or tries to know, every body blames me for these behavior and people nearby me feels that, I have generally developed this habit of getting irritated.

When ever I try to speak that I am not feeling well or my mood is very bad, they would in turn blame me like "you are this kind of person only", rather now a days I am getting irritated inside.

It so happened sometime back that, there were some lines in my mouth which I mistook it as leukoplakia and read that of being cancer, I refered to the local doctors and even seek the help of these website, and all of them told me not to worry, but till today I am so afraid that I have quit smoking but again I keep on asking my self that what has happened to me, m I so ill that I have quit WHY? I do not have any answer, these thoughts are always killing me.

Also an e.g like while Driving or riding I feel that I am unbalance and going to bang somewhere.

Why this is happening, I was a very jovial person, used to live life king size, married intercaste(love) all was so good when I look back but today I feel deserted and desperate.

Will these medicine help! I have taken 1 dose in the night and in the morning but can't feel anything.

Need your extensive help!

Thank you.

Brief Answer:
cognitive behaviour therapy indicated.

Detailed Answer:
Dear XXXXXXX
Its unfortunate that no one around is paying attention to your problem.

When a person is depressed, everyone and everything around is perceived negatively.
May be thats why, you misinterpreted lines in your mouth to be cancerous.

That imbalance during driving must have been due to thought ruminations. The mind in depression is preoccupied with chain of negative thoughts. This leads to impaired attention and concentration.

Biological explanation to this is neurochemical imbalance in brain.

You will be fine for sure. The drugs will work for sure. Just keep them taking regularly.
If a particular drug does not work, an increase in dose or change of drug will be needed.

If possible, try and find a clinical psychologist near you. Apart from drugs, cognitive behaviour therapy is needed in your case. This will modify your thoughts and perception about self and world.

Hope this helps.

Feel free to ask further. I am always available to answer your further queries.

Take care.
Thanks.

Brief Answer:
repirts absolutely normal, continue drugs.

Detailed Answer:
Dear XXXXXXX
I have gone through your reports.

All the parameters are within normal limits.

In a depressive, usually all the basic blood investigations are normal.

So, I would suggest you to continue with your drugs and lifestyle modifications mentioned in our earlier discussion.

If there is no response after two weeks of taking drug, consider increasing the dose.

If pissible, do consult a clinical psychologist for cognitive behaviour therapy.

I wish you speedy recovery.

Kindly close the discussion if you are satisfied with the response and give your valuable feedback.

I am always available for further clarifications.

Regards.
