Suggest Treatment For Mass In Right Testis

Question: Hi
I am a 33 year old male. I have been diagnosed with a mass in my right testis although all the tumor marker tests have been showing negative. MRI scan has been done which shows mass in right testis but does not give any clear indication of anything although doctors are 99% sure its not malignant.
Now, my doctor ( consultant XXXXXXX ) is suggesting me to undergo a surgery where they will examine the testis, do a on-spot biopsy and then if determined as malignant, then are suggesting removal of tests.So far, all the indications are pointing that my case is non-malignant. I am worried and would want to know if surgery is absolutely necessary. Is there a non-surgical way to find out what is the mass and would it be dangerous to leave it as it is.

Hi,

Welcome to Healthcaremagic.

The procedure being planned for you is the protocol which is followed for testis mass and it needs to be done for you because of following reasons
-Tumor markers in testis cancer are not 100% . They are not used to diagnose cancer . It may or may not be raised.

- cancer from non cancer mass is differentiated by only microscopic study of the tissue . This is 100% accurate. No other methods are so accurate .

-Usually anywhere else in body we take a biopsy or tissue material using a syringe which we call FNAC. But for testis mass FNAC is not done to avoid any spread of cancer if there is little suspicion of cancer also.

-Firm to hard mass of testis at your age, cancer needs to be ruled out.

I suggest you to go ahead with the surgery and if during on spot biopsy(frozen biopsy) , if it is non cancer get the mass removed only and if it is cancer get the testis removed and further treatment depending upon stage.

The ill effects of keeping the mass without surgery and frozen biopsy are
- if it is cancer it will spread and difficult to treat later.
-if it is non cancer it may still grow to more size and may cause cosmetic problem or risk of it to go for cancer in later life.

Thanks doctor. I will go ahead with the procedure. I have few more doubts. Post surgery how long i need to take rest and the mass on my right testus is not very hard or growing.my beta HCG levels are also zero.so how good is my chance of having no cancer.my XXXXXXX is 99 percent sure its not malignant but intetested to have a second opinion.

Chances of being non cancer is 95-96%

Hi,

After surgery you need to take 2 weeks rest. After that you can do office and desk work. Don't lift heavy weight for 2 months.

Since mass is not hard and Hcg level is zero then the chances of being non cancer (non malignant) is 95-96% .

Thanks doctor. I am having low sperm count and during analysis we found out this mass on the testis. My consultant XXXXXXX says that the low sperm count may be in a way related to this mass and once removal of this the sperm count has a chance of getting back to normal if there are any blockages etc

If the cause of low sperm count is blockage in the path due to the mass then the count will become normal after surgery.

If the cause is genetic or hormonal etc then it won't become normal unless measures are taken for that.
