Was it an ischemic stroke? what dod CT scan and MRi reports say?
TPA is not always indicated sometimes there are factors which can make TPA, a death sentence for patient like hemorrhage , hypertension, SOL brain old age etc. And there are a few situations when TPa has no benefit.
In my opinion doctors would have considered all the possible options and there must be a reason that they didn't go for clot resolution. There is always a weighing between benefits and risks and I am quite sure risks would have outwieghed the benefits.
I would like you not to feel bad about what happened In my opionion experts would have made the choice for a reason and reason is always patient's short and long term benefit nothing else.
I would also mention here that if he is following commands and moving limbs then there is every chance of him getting off the vent real soon so keep hopes and morale high.
Things will get better , I am glad that your neighbour found him in time and he is on road to recovery.
