Brief Answer:
I suggest to meet an Infectious Disease Specialist.

Detailed Answer:
Hello! I have been through your question.

First you should know that mites are small arthropods with eight legs, and only two kinds of mites actually live and feed in human skin, the scabies mite and the hair follicle mite. So, what kind of arthropods you have?

Related your concern if these are scabies mites you may experience itching after treatment with permethrin cream but this is not a sign of treatment failure. However if living mites are seen after 14 days, retreatment is necessary.
Furthermore you should know that for people with altered immune systems, for people who have crusted scabies, or for people who don't respond to the prescription lotions and creams (like you) usually it is recommended to use Ivermectin. During this time you should use antihistamine and smoothing lotion (calamine lotion) to relief from itching.
In your case I suggest to meet a Infectious Disease Specialist .

I hope my answer helps you.
Feel free to ask me further.
Regards.
