Suggest Treatment For Migraines, Headaches, Double Vision And Soreness Around Eyes

Posted on Mon, 16 Mar 2015
Question: My problems started with a headache every day. I have a history of migraines but these were not migranes. In about a month I developed double vision and soreness and tenderness around my right eye. The head pain is horrible but the double vision has subsided. The superior oblique eye muscle is not working. MRI revealed 3rd cranial nerve is inflamed.

Detailed Answer:
Hi XXXX,
Thanks for being on healthcaremagic.com.
I am Dr.Ajay Panwar,a neurologist,here to answer your query.

If you would have presented to me as an outpatient,my clinical explanation would have been-
Migraine like headache episodes with consequent double vision, which improved later on can occur due to the following causes-
Ophthalmoplegic migraine
Vasculitis
Aneurysm

MRI revealing 3rd cranial nerve inflammation can be interpreted as arachnoiditis.
However, without having a view at the MRI,it will just be incomplete and not rationale to give any opinion.
So,please upload MRI images and report.
Also,please tell a few details-
1)Are you a diabetic or hypertensive?
2)Any past history of Tuberculosis?
3)Did you have swelling or proptosis of your eyes?
4)Did the double vision improve on itself or any treatment?

Waiting for your follow up question.

Dr.Ajay Panwar,
MD,DM(Neurology)
