Suggest Treatment For Mild Chest Pain While Resting

Question: I have mild chest pain from time to time - usually while resting. It can wake me during the night. I have had mild hypertension in the right side of the heart in the past.

Regarding your concern, I would like to explain that when dealing with a chest pain, a comprehensive differential diagnosis is necessary.

First of all, regarding chest pain characteristics (appearing usually while resting and not exercising), it doesn't seem to be cardiac related.

Nevertheless, in front of an organic chest pain (wakes you during the night), three acute major chest pain syndromes should be ruled in/out: aortic syndrome, acute coronary syndrome (which seems to be less likely) and pulmonary embolism.

Regarding mild hypertension on the right of the heart, could you please confirm if you mean an increased arterial pulmonary hypertension?

If such is the case (increased PASP), then a differential diagnosis between a chronic pulmonary (bronchial) disease and pulmonary embolism should be done.

Other possible causes of chest pain are: backbone degenerative disorders, costo-chondritis, pleural and pericardial inflammations, shingles, cholecystitis, etc.

Fot appropriately differentiation of these numerous potential etiological factors, it is necessary a careful physical examination, resting ECG, chest X ray study, cardiac ultrasound, several lab tests (complete blood count, PCR, liver and renal function tests, cardiac enzymes, etc.).

If sufficient suspicions are raised, more specialised imagine tests, like chest angio CT & MRI may be helpful.

You need to discuss with your attending physician about the above mentioned issues.
