Question: I have been ill for 7 weeks. No energy, weight gain 19 lbs., big knot in the right side of my neck, headaches, muscle aches, all joints hurt. I have trouble lifting myself out of bed. My right shoulder is numb to the touch and aches really bad. When I eat anything my stomach instantly swells looking like I am nine months pregnant. I have always been active, with 10 grandkids. Now I don't have the energy to do anything, anything at all. Even small tasks like grocery shopping is not an option, it wears me completely out walking from the bedroom to the bathroom. Any suggestions?

Hi, thank you for using Healthcare Magic. This sudden change in the last 7 weeks is worrisome. If you were my patient, I would want to see you in my office very soon and do a complete physical examination. I would then do bloodwork and depending on other factors send you for imaging studies or endoscopy to see what's going on in your stomach.

Things that come to mind with all your symptoms include hypothyroidism, diabetes, a partial obstruction in your stomach, anemia, a collagen vascular disease such as lupus, or even cancer. You could have picked up a virus that is hanging on and causing fatigue and these other symptoms. I would want to know exactly what the big knot in your neck is, what is causing the shoulder pain, and be sure your heart and lungs are working okay.

All this requires a visit to your doctor and a full workup. I saw in another question you asked about burping back food. This can happen when a sac develops in the esophagus. Food is caught in the sac and later can be burped back up or thrown up. (I had a patient with this once.) This would be diagnosed by an endoscopy or barium swallow x-ray. You have a couple of good reasons to get an upper endoscopy. The specialist who does this is a gastroenterologist.

I strongly recommend you see your doctor soon and get the physical examination, blood tests, and studies of your stomach. Once your doctor knows what is going on, she or he will be able to treat you and hopefully you will start feeling better.
