Question : Hi I have not gotten any better, there has been a couple days where the pain was almost gone. Because it comes and goes and because I feel muscle spasms in my groin and my left testicle is more distended when in pain, I'm thinking I have a pelvic floor problem or a hernia.

Brief Answer:
Get a scrotal ultrasound

Detailed Answer:
Good Day and thank you for being with Healthcare Magic!

I would encourage you to get a scrotal ultrasound to check for inguinal hernia or to diagnose orchitis.

A urinalysis would also be useful to Check for UTI and if it is abnormal then we will proceed in doing a culture and sensitivity to identify the bacteria and treat it with a proper antibiotic.

Are you taking any pain medications? NSAIDs will help a lot for the pain and swelling.
