Question: Presenting with General malaise, nausea, lower back and bilateral leg pain, chills.....37 year old male living in Jamaica. Saw a MD on Nov 14th, after 3 days of vomiting and lower abdomen pain diagnosed with acute gastritis, tested negative for presence of h. Pylori....given intravenous dose of Nexium, pain subsided. Left with scripts for motilium and protonix, any idea what the problem may be?

Hello,

As soon as I hear these symptoms I think of a kidney infection (pyelonephritis).

I would get a CBC with differential, urinalysis, and send a urine for both culture and antibiotic sensitivity testing if the urinalysis is indicative of infection.

Many of the symptoms you described are consistent with pyelonephritis:
malaise
low back pain
chills
nausea and vomiting

I'd also get an abdominal X-ray to look for kidney stones (and generally to look at abdominal organs) that could increase the chance of pyelonephritis.

Another thing to consider is appendicitis although usually pain is more abdominal.

It's possible the Nexium IV made him feel temporarily better because of the bolus of fluid rather than the Nexium itself.

If you've already ruled these things out, please write back and we'll trouble shoot other ideas.
