First line therapies for neuropathic pain include two main groups. One group is the one including anticonvulsants. The two main alternatives in this group are Gabapentin and Pregabalin.

The other main group are the antidepressants. The main alternatives are Duloxetine or Amitriptyline.

If these alternatives do not work opiates are usually the main alternative which could be added.
