Detailed evaluation and Consultation

Detailed Answer:
Thanks very much for your question.

Let me begin by saying this sounds like a complicated neuropsychiatric issue, and I think we would get more out of this exchange if you were to ask me at my private direct link below, so as to allow us more time to discuss this. But I will give you an appropriate answer for now below.

You say your husband experienced a stroke 4 years ago, and has since been exhibiting signs of possible personality changes, such as strict habits and strong stands, and wanting things a particular way or getting upset.

It is entirely possibly this is a neuropsychiatric manifestation of his stroke. Strokes, especially Frontal lobe strokes, can certainly be associated with personality changes. Often this manifests as disinhibition, or behaving inappropriately and more emotionally than usual.

It sounds like your husband wants things a particular way and reacts badly when they do not fit his liking. I'd like to see a copy of his MRI report if possible to localize his stroke, but to answer your question, yes it is possible this is caused by stroke.

The treatment is the same whether this is a psychiatric or neuropsychiatric issue -- he may benefit from a low-dose neuroleptic or antidepressant.

I just want to say "thank you". This makes so much sense to me. We are starting him with a new doctor, my physician, in January, as he has transferred his insurance from Keyser to the plan I am on.
It gives me an understanding to help not to get frustrated, and I will be keeping this information to share with his doctor. If I don't see he gets help, I will be contacting you. Bless you.
