Brief Answer:
Baby might be suffering from kidney disease.

Detailed Answer:
Thanks for posting on health care magic.

Rbc cast, granular cast indicates pathology of kidney. Your baby is most probably suffering from kidney disease which require very urgent evaluation.

Your baby needs an ultrasonography KUB, a DTPA scan to detect any congenital anomaly. If urine culture shows infection child should be started on antibiotics.

I feel you should see a pediatritian at the earliest and if advised get your child admitted in a hospital where there is a good renal unit. The investigations will get done easily and evaluation will be easy.

Thank you.
