Brief Answer:
prilocaine could help

Detailed Answer:
Hi
The problem you have outlined is not an unusual one, especially among uncircumcised men and as a matter of fact the glans sensitivity becomes very high soon after circumcision till it acquires a protective layer a couple of months later.
In your case you can try gently retracting the foreskin and applying an ointment like Prilox or 5% xylocaine ointment (not the 2%gel which is more commonly available). Both these should be easily acquired in good well stocked pharmacies but the effect takes over 30mins to begin and is temporary.
Because you find your forekin relatively tight, it might not be a bad idea to have a Circumcision and wait for a few weeks for the glans to lose its sensitivity.
Hope I have been of assistance.

Brief Answer:
none for long term

Detailed Answer:
Hi
I am afraid there is none that will give a long term benefit. If there was, I would have recommended it earlier. That is also the reason why I recommended Circumcision though it is not absolutely indicated.
You can try the cream which I have mentioned and then take a call.
Regards.
