Question : I have both RA and OA. However, my left thumb will suddenly (as in all is well before I sleep and far from well when I awake) swell, stiffen, become tender to the touch and movement is inordinately painful. Neither ice or anti-inflammatories seem to have any salubrious benefits.

I have gone through your query. Your problem can be most likely due to acute gout attack. Checking blood levels of uric acid will be helpful to rule out this. Colchicine therapy will be helpful to resolve the acute stage. And after it is resolved you can switch on to drugs like allopurinol.
